YOUTH RECREATIONAL SOCCER & T-BALL/BASEBALL
COACHES REGISTRATION FORM

PLEASE SUBMIT BY Jan 26th for us to be able to make sure we have enough coaches.

To make these program work we need coaches. Coaches are a big part of our Soccer & T-Ball/Baseball Program. We also have set up a structured plan for coaches. With this plan Coaches do not need to have experience.

Coaches will receive at end of Season $65. (must be present 8 out of 10 games to receive this)

Coaches will be able to register their child(ren) before it is open to the public. (If this submission is received before Jan 26th.)

Coaches will need to be a member to be able to coach and be under the Centre's insurance and also must get a Vulnerable Sector Check (please see at bottom to download and print).

Soccer will be on Monday Evenings & T-Ball/Baseball will be on Tuesday Evenings
